I think some ports are missing from your list. With these ports Nated I still get disconnection issues. Yet if I most my computer to my DMZ it's sweet.
see: Connection to the Game Server Has Been Lost.

So if I enable this, what does it do for me?
Will this stop my disconnecting every ~5 minutes?

I tried in DMZ with the same result.
Just every fine minutes or so I am disconnected from the server.
Wouldn't be as bad if it were every once in a while, but it's every game.

But enabling these ports won't fix the problem?

PortForward.com - Port Forwarding Guides Listed by Manufacturer and Model

There's a good guide on there for most routers/games.

I hope this helps anyone that isn't sure how to do it.
